of our physicians participate in numerous insurance plans, including
HMO's, PPO's, Medicare, and Medicaid. If you are unsure
if our providers are on your plan, please call our office at 815-937-5200
to inquire. We will be more than happy to find out the
needed information.
Although
we do participate in numerous plans, you are required to pay the
full amount of your office visit, unless your insurance card indicates
otherwise. If you are unable to pay at the time of service,
other financial arrangements will be made. However, if your
insurance card indicates a co-payment or deductible, you are responsible
for that amount.
